| Description: | Glacier Hills Care and Rehabilitation Center has been providing compassionate quality care and excellent customer service to our residents for more than 36 years. To enhance our approach to person-centered care, we are seeking three (3) Admission/Discharge Nurses (RN or LPN) for our 80 person sub-acute rehabilitation residents:
We have one (1) casual status position available for the following shifts:
Every Saturday and Sunday, 12:00 pm – 8:30 pm

| Duties: | Coordinate admissions with Admissions Representative
•Completion of all admission paperwork, including:
o Physical assessment
o Physician orders
o Care Plan
o MAR’s
o Treatment Sheets
o Initiation of Flow Sheets
o Dietary Notice
o Completion of Admissions Checklist
o TB Testing
•Coordinate Discharge Planning with Social Work
•Completion of discharge paperwork
•Educates resident and/or their designee regarding transfer and discharge plans
•Provide floor nurse duties as needed during non-peak times

| Qualifications: | •Degree in Nursing from an accredited college.
•Possess a current and clear RN or LPN license in the State of Michigan.
•12 Months related experience in a hospital setting, or
•A minimum of 6 months experience in a long-term care or rehabilitation facility.
•Must possess excellent communication skills, have a special interest in working with and caring for older adults, be compassionate, reliable and a team player
